The alpha-blockers are an excellent choice for patients with diabetes or insulin resistance, because these drugs improve insulin sensitivity and favorably influence all the lipoprotein fractions. They also reverse or minimize the symptoms of prostatic hypertrophy and bladder outlet obstruction in men. Diuretics block the excretion of calcium in the kidneys, which makes them attractive for perimenopausal and postmenopausal women. If a patient's BP is 180 110 mm Hg or higher, it is realistic to anticipate the eventual need for more than one antihypertensive drug. A second agent can be added when the dosage of the first has reached at least the middle of the recommended dosing range and has been taken for six to eight weeks.
